The provisions of this agreement are drawn to the attention of the Indemnifying Party where the
Consumer Protection Act 68 of 2008 applies to the relationship between MiXchange Learning Hub,
MiXchange Ministries, Inhloso Heritage Foundation, and Heritage of Faith Ministries International
(hereinafter “the indemnified parties”) and the Parent and/or Guardian of the minor child/children
(hereinafter “the indemnifying party”) The effect of this agreement being that the indemnifying party
may have limited or no recourse against the Indemnified Parties in the circumstances referred to
herein.

I hereby state that I have wilfully chosen to take part and have the minor child/children take part in the
Community Learning Hub field trip to the Grove Ice Rink, Pretoria, on Friday, 21 November 2025,
offered by the Indemnified Parties (hereinafter “the Activity”).
I, as the Parent/Guardian of the minor child/children, acknowledge and understand the inherent risks
associated with the Activity. I further understand that the Indemnified Parties, their officers,
employees, and agents will not be and/or are not responsible for any injuries, property damage, or
liability that may arise from the Activity. I further assume full responsibility for the decision and the
consequences thereof, to allow my child/children to take part in the Activity as set forth herein.
I do hereby release and indemnify the Indemnified Parties, its officers, employees, and agents from
liability in relation to any and all costs, losses, expenses, damages, claims, suits, causes of action or
any other liability or responsibility whatsoever, in law or in equity, including attorney’s fees and
related costs, resulting from any injury to any person(s) or damage to property arising out of, or which
may in any manner be connected with the Activity and my child’s/children’s participation therein,
whether suffered by me or any other third party, and accordingly will not hold the indemnified parties
liable.
